It works within the Admin site, but does not appear at all in the Live site -- I get a screen asking me to login. I have provided the Anonymous role with permission to Create * but this does not solve the problem either. Any ideas would be appreciated.

If your site isn't live, you could try to give * rights to the anonymous user, just for the sake of debugging.

We have a feedback form working in 3.1. Apart from overriding the collectedinformation.tpl we just had to give read permissions for objects of that feedback class.
